The following documents are required for eligibility:

• Proof of Age – Child must be 3-5 years old to participate
  
  
• Proof of Adress (i.e. Utility Bill)
  
  
• Proof of Income
  
  
  • For Part Day – Income earned in the last 12 months prior to registration (W-2, Income Tax Return, Check Stub & Statements)
    
    
  • For Full Day – Income earned in the month prior to registration (Proof of employment, school or training. Both parents must be working or in school with no caregiver at home)
    
    

The following are required for admission:

• Current Year Tuberculosis clearance skin test result
  
  
• Current immunization records with authorized signature or clinic stamp (Polio, DTP, MMR, Hib, Varicella and Hepatitis B)
  
  
• Physical Exam within 30 days of start date
  
  
• Dental Exam within 45 days of start date
  
  
• Copy of Child’s Health Insurance card
